Connecting you to your community

Life can be challenging when you live with a disability or a long-term or life-limiting health condition. These challenges can feel even more overwhelming if you live alone, feel isolated, or find that your health is affecting your confidence, wellbeing or family life.

DisAbility Connect provides personalised, community-focused support to help you feel more connected, more confident and better supported in your daily life. Our Social Prescribing+ approach helps you access meaningful activities, community resources and services that support your independence, wellbeing and overall quality of life.

What the project can offer

Using our in-house Needs Assessment and Personal Progress (NAPP) tool, we explore all areas of your life to understand your needs, strengths and goals — creating a personalised support plan so you get the right help at the right time.

Support to access wellbeing activities, social groups and local community opportunities.

Help with confidence-building and reducing feelings of loneliness or isolation.

Information and guidance on aids, equipment and home adaptations.

Support to access cost-of-living resources such as warm hubs, low-cost meals and food support.

Help with transport arrangements for appointments or community events.

Assistance attending first-time activities to ease anxiety and build confidence.

Support with reconnecting to hobbies, interests and social networks.

Referrals to internal Disability Solutions teams when additional support is needed.

Coordinated links to trusted external organisations for specialised support.
